Media Press Kit for FACT Accredited Organizations

 

The Foundation for the Accreditation of Cellular Therapy (FACT) encourages FACT accredited organizations to publicize their accreditation, and provides tools to help communicate this accomplishment to patients, providers, insurance carriers, and other community stakeholders.  Organizations receiving FACT accreditation will receive a list of strategies to publicize their accreditation, a sample press release, and an electronic copy of the FACT Seal of Accreditation upon successful completion of the accreditation process.  The Media Press Kit is a recent addition to the supplemental material FACT provides to all accredited organizations.  If your FACT accredited organization would like to obtain the Media Press Kit materials prior to your next reaccreditation cycle, please complete the FACT Seal of Accreditation License Agreement below. 

FACT Seal of Accreditation License Agreement

The License Agreement must be read in its entirety and signed by an authorized agent of the accredited cellular therapy program or cord blood bank before the FACT Seal of Accreditation may be used. The last page of the License Agreement must be either scanned and emailed to Jill Hempel, Quality Assurance Coordinator, at jhempel@unmc.edu, or faxed to the FACT Office at 402-559-1951. Upon receipt of the completed License Agreement, FACT will send a link to the media press kit materials including copies of the Seal which may be downloaded for use by the accredited Cellular Therapy Program or Cord Blood Bank.
